What this application does

Premier Solutions Health Connect is a backend data-integration application that connects to the Practice Fusion electronic health record (EHR) system using the HL7® FHIR® R4 standard and the FHIR Bulk Data Access specification. It retrieves clinical and administrative records from practices that have expressly authorized the connection, for the purpose of practice operations, care coordination, reporting, and analytics on behalf of the authorizing practice.

Information we access

With the authorizing practice's approval, the application may access FHIR resources including patient demographics, encounters, conditions, medications, allergies, immunizations, laboratory results and other observations, procedures, care plans, clinical documents, and coverage information, limited to the scopes granted by the practice in the EHR.

How data is used

• Data is used solely to provide services to, and at the direction of, the authorizing healthcare practice.
• We do not sell patient data.
• We do not use patient data for advertising or marketing.
• We do not share patient data with third parties except as directed by the authorizing practice, as required by law, or with subcontractors bound by equivalent confidentiality and security obligations.

HIPAA

Protected health information (PHI) accessed by this application is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Where we act as a business associate of a covered entity, we do so under a business associate agreement (BAA).

Security

Connections to the EHR use OAuth 2.0 / SMART Backend Services authentication with asymmetric key cryptography over TLS. Exported data is stored on access-controlled systems, encrypted in transit and at rest, and limited to personnel who need it to perform services for the authorizing practice.

Data retention and deletion

Data is retained only as long as needed to provide services to the authorizing practice or as required by law. Practices may revoke this application's access at any time from within Practice Fusion, and may request deletion of previously exported data by contacting us.

Patient rights

Patients seeking access to, amendment of, or an accounting of disclosures of their health records should contact their healthcare provider, who remains the custodian of the medical record.

Changes to this policy

We may update this policy from time to time; the current version will always be available at this URL with an updated effective date.
